Steve please help! Your info was the most helpful to me so far. We have a Professional Corporation in Indiana which consists of currently 1 physician (another to be added within 4 months). To date we just use the business tax ID for most insurance billing for this provider (unless the insurance companies have assigned another provider id#). I have a type 1 NPI# for this physician I guess (individual#). From what I understand, that b/c it's a PC (incorporated from the start in May '05), I should have a separate NPI for the "corporation" as well? The physician (although currently the only one) is an employee of the PC. I asked 3 other clinics, 2 were PCs that weren't sure of anything & so were getting both #'s just in case-and 1 was a LLC that said they have a separate NPI for the "clinic" and each Dr has their own individual NPI#. They also have a "clinic" medicaid # they said along with each Dr's own individual medicaid# (we don't have a separate medicaid# or anything for the PC- except the CLIA license). So if I am supposed to get a separate NPI for the PC (please advise), when I look at the site to get the NPI# - do I pick the 2nd option of NPI for health plan? That makes no sense to me since the PC is not a "health plan" & it doesn't have a separate special license or anything. What do I put for the "identifiers" other than the tax ID? Under the Dr's individual NPI# profile I was putting the different provider #'s. Also- the insurance companies have been asking for the NPI#, so I've been giving them the individual # that I already have for the physician. Is that wrong? I called the NPPES # a couple of times re: all the above & rec'd different answers- they were not any help, & then they said to call the insurance companies to ask what they want! Any info would be most appreciated!
