And just an FYI, carriers are now saying you can only perform certain tests every X amount of months etc. They issue "clinical policy bulletins" which you agree to when you sign your contract. One insurance co. only allows one full echocardiogram per year, otherwise they will only pay for a limited echo. Another will not allow nuclear stress tests more than once per year (even though the patient had a MI and has symptoms---ok, we appeal that). etc. etc. I feel like a circus animal jumping through so many hoops.

We have to check for clinical policy bulletins on every major carriers websites every month. Despite signing up for notification via email and the steadfast snail-mail, we rarely get the notification of new policies.
