My first question is can you apply for a NPI for the psychologist? That should go in the rendering field. My thinking is the temporary permit will be used as the "license" and you will need an NPI. The rendering should be the psychologist and the referring should be the licensed psychologist on the form.

If anyone else has any input please do, I have not dealt with anyone with a temporary permit just supervision for a lower level provider.
