Looks like this has been happening for quite some time now... because my doc who works outta New York had been experiencing these problems and decided to send all procedures (trigger point, MBB, ESI, RFA etc / injections along with medical records / notes etc, when I asked him about it he said most claims always came back asking for additional documentation and this delayed his cash flow; now he submits all injections with medical records; his latest statistics on commercial claims are 36.21% get paid in 0-30 days and 53.57% get paid in 31-60 days that's almost 90% paid in 60 days... on his total claims almost 94.51% get paid in 0-60 days.
