All you need is a simple form with your practice name etc.. on it. With something that states that the patient "name" is paying in full for services or treatment rendered today and will not seek any reimbursement from their insurance carrier. They are getting ___ % off and the dollar amount for paying in full today. Have them sign and date it give them a copy.

So basically a simple form letter that they sign. File it or scan it into their chart, have an internal code in your software program that makes the adjustment for payment at time of service.

It's really quite simple you don't have to create a lot of paperwork for it, and you get paid right away. Many offices don't even use a form but you can and you'll have their signature if there is a problem.
