Then, to answer your question in a general manner - Medicare is a federally administered program; however, they contract the job out as the government tends to do. "Fiscal Intermediaries" are contracted to handle all part A, ESRD, Indian Health and such, "RHHIs" handle hospice and home health, "Carriers" handle Part B, and "DMERCs" handle DME. These are further divided by region. CMS makes the majority of the rules for Medicare, but each contractor is given alot of leeway in some areas. Furthermore, it is the FI/Carrier's responsibility to distribute bulletins, program changes, etc., not CMSs, so you need to be in close touch with whoever our contractor is. Also, each of these FIs and Carriers have wide discretion in alot of areas such as coverage, so, once again, you will need to have their website on your bookmarks.
