The fact that you got "yet another" referral to the manual might be a clue.

You may call it useful, but I learned Texas, Oklahoma, and Indiana Medicaid SOLELY from their manuals. Each time I had to bill Medicaid in those states, there was no one to ask, and I didn't even have internet access. I learned EVERYTHING I needed to know from those manuals. And you know what? Each time I had a backlog of at least HUNDREDS of denied and rejected claims dating back at least ONE YEAR and each time the first thing I did was take those manuals home and read them from start to finish. And guess what? About 90% of the claims I appealed/resubmitted got paid the first time. I managed each time to clean up HUGE backlogs within a few weeks' time. Not because I am such a whizbang biller (well, I am, but that's not the point ;) ) but because I did something none of the people before me bothered to do - I read the manuals.

I was answering in a joking manner, but I was NOT joking when I said Medicaid is the easiest thing to bill. But you've got to do your homework first. The reason everyone is afraid to touch Medicaid is because they're too impatient to read the manual.
