The claim is automatically crossed over to Blue Cross and Blue Cross pays 5.00 and states there is no further responsibility due. Account has balance of 33.00 and patient is refusing to pay the balance because Blue Cross states they have no responsibility. Doctor is contracted with Blue Cross. Is the balance a write off?

I am wondering if anyone else has run into a problem when their physicians see patients at a nursing home that has no NPI. We have not even attempted to bill the insurances because NPI#s are required. The facilities consider themselves small and deal with billing the patients directly for any care they receive. Thats not how things work. We dont invent a diagnosis to go with a CPT code.
In billing and coding, the diagnosis is what is selected by the provider that corresponds to what is wrong with the patient.
We,as billers, dont code, and again, we dont invent codes if one is not provided by the provider.
